Advanced Health Directive
You may not always be in a position to make your own decisions about the medical treatment that you receive. Inability to make decisions or inability to communicate those decisions to others may prevent you from receiving your preferred medical treatment.
An Advanced Health Directive is a formal way of giving instructions for your future health care and will activate only when you are not able to make those decisions yourself. You can also include personal information that health professionals should know, such as the existence of allergies or religious beliefs so that medical staff can make infirmed decisions about the types of medical care you receive.
An Advanced Health Directive will also allow you to appoint a person or people (your “attorney(s)”) to make those decisions on your behalf.
You may use your Advanced Health Directive in a general way, such as stating that you wish to have all available treatment for your condition regardless of personal cost, or in a more specific way, such as the specific exclusion of medical procedures or circumstances in which medical treatment should be withdrawn or withheld (such as where there would be little or no chance of your recovery).
